Doctors use something called Patient Need Groups (PNGs) to help them understand how much care each person might need.
Think of it like sorting people into groups so doctors can help everyone in the best way possible. We use the Johns Hopkins System to generate a score for each patient

🟢 Green Group – Low Need
- People here are usually healthy.
- They might only need check‑ups or simple advice.
🟠 Amber Group – Medium Need
- People have some long‑term health conditions.
- They need more regular support so they stay well.
🔴 Red Group – High Need
- People have more complex health problems.
- They need more help, more check‑ins, or support from different health teams.
🔍 How Do Doctors Decide the Group?
Doctors don’t pick the group themselves.
The John Hopkins Tool looks at things like:
- Hospital visits
- New illnesses
- Medicines changed or added
Then it updates your group every month.
This means your group can change if your health changes.
