New Patient Registration

Please note you will remain registered with your current GP until we confirm your registration is complete. - Current wait time (Mar 2024) to process registration requests is 4-6 weeks

To make the process smooth please include your full address including house names, apartment or room numbers as Primary Care Support England (PCSE) will  write to you to confirm your registraiton.   If they are unable to verify your address with the Royal Mail due to missing information they will reject the request to register at the practice.

As part of our ongoing service to patients: If you are aged under 75, where the Doctor has not seen you for three years or more and you request an appointment, an appropriate check-up will be provided. If, however, you are 75 or older and have not been seen for one year, we will offer the same service.m. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about yourself across the internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

 

Named And Accountable GP

Under the terms of the latest GP contract, the Practice is required to allocate all patients a “named and accountable GP”. This is purely an administrative exercise as all of you will already have a named GP.

It is not essential that you see your "Accountable or usual GP" at all times, but we would encourage you to form alliances with at least two GPs within the Practice which will allow more flexibility when booking an appointment.   It is accepted however, that with the drop in clinic arrangements where the clinics are run by our Nurse Practitioner and our clinical Pharmacist that this is not always possible.

If you wish to know who your named GP is, kindly ask at reception and the practice staff will be happy to let you know.   

Please note that your named GP will not be available at all times and if your need is urgent, you may need to discuss them with an alternative doctor.

 

Training Practice

This practice has been accredited to train various clinicians and doctors, is regularly inspected and only doctors approved for this purpose will carry out an  assessment.

Sometimes the doctor may wish to video their consultations and patients may therefore, be asked to sign a letter of authorisation to permit this. Your permission will always be requested beforehand.
