How to register with the practice

In order to register at Northgate Medical Practice, you need to live in the area covered by this Practice, as shown in the map via the Practice Area link on the right. You should bring your NHS medical card, with your new address completed and the document signed, to the surgery where you should book an appointment with our Nurse for a Health Check. If, however, you do not have a medical card, please ask the Receptionist for a registration form and provide proof of your current address.

As part of our ongoing service to patients:- If you are aged under 75, where the Doctor has not seen you for three years or more and you request an appointment, an appropriate check-up will be provided. If, however, you are 75 or older and have not been seen for one year, we will offer the same service.
